Through our Center for Connected Care,you will helpprovide a full continuum of careby processing inpatient referrals, coordinating discharge plans, coordinating services and more.Your efforts directly enhance the quality of life for patients and their families every day.
A caregiver in this position works8:00am to 4:30pm with every 4th weekend and 2 holidays per year (1 summer and 1 winter). This position requires travel to East Cleveland Clinic hospitals and facilities.
A caregiver who excels in this role will:
- Act as a member of the interdisciplinary team for the assigned functions related to the coordination of services with internal and external referral sources.
- Processinpatient referrals into hospice, ensurecontinuity of care and coordinatedischarge planning in contract facilities.
- Actively educate potential customersaboutthe benefits and appropriateness of hospice care.
- Ensure patientsand their familieshaveadequate knowledge and an understanding of the hospice benefit.
- Process inquiries for all new referrals (internal and external).
- Ensure a seamless transition from hospital to the community setting byorderingmedical supplies and DME,handingoffcommunication with the field team and documentinginthe electronic medical record.
- Perform assessment of potential patients consistent with Cleveland Clinic Hospice policies,proceduresand admission criteriaalongside thephysician.
- Perform admission assessments for new admissions into contracted facilities and document information in the EMR withinspecifiedtime frame.
- Planthedelivery of patient care in collaboration with other disciplines to provide comfort and symptom management.
- Perform daily assessment and coordinatethe plan of care withinpatient staff for General Inpatients (GIP).
- Assesspatientfor safety risks,abuseand neglect.
- Assess and document pain usingtheappropriate painscale and managepatient’spain.
- Documentchangesin patient condition and effectiveness/timeliness of care/interventions.
- Evaluate,updateand revisetheplan of caretofacilitateachievement of expected outcomes.
- Know action, usual dosage,indicationsand side effects of medications administered.
- Attendinterdisciplinary meetings as assigned.
Minimum qualifications for the ideal future caregiver include:
- Graduate from an accredited school ofProfessionalNursing
- Current state licensure as a Registered Nurse (RN)
- One year of current clinic experience as an RN
- One year of experience in an acute med/surg or critical care setting
- Basic Life Support (BLS)Certification through the American Heart Association (AHA)or the American Red Cross
- Knowledge of JCAHO and Medicare standards and regulations
- Ability to work irregular/varying hours as needed
- Ability to travel to hospitals and community referral sources
Preferred qualifications for the ideal future caregiver include:
- Bachelor’sofScience inNursing (BSN)
Physical Requirements:
- Requires full range of body motion including handling and lifting packages, manual and finger dexterity and eye hand coordination. Occasionally lifts and carries items up to 50 pounds. Requires corrected vision and hearing to normal range.
- Requires standing and walking for extensive periods of time.
Pay Range
Minimum hourly: $30.41
Maximum hourly: $46.38
The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.).
